If you would like the office to file insurance on your behalf, please furnish all relevant insurance cards before your first session. 

You can find answers to questions about authorizations, deductibles and co-payment information by calling your insurance company using the number found on your insurance card or located in your current plan booklet. I participate in Anthem, BC/BS, Cigna, United Healthcare and UMR. 

PLEASE HAVE ALL INFORMATION PROVIDED TO YOU FROM YOUR INSURANCE COMPANY FOR YOUR FIRST APPOINTMENT.         

By calling your insurance company BEFORE your first session. You will have factual information about how much you will be expected to pay per session. This will assist you in your financial planning and utilizing your sessions to obtain maximum benefit.   

 Acceptable forms of payment include cash, check, HSA and credit cards. All copayments, co –insurance and deductibles are to be paid in full at the time of service.
